Your health is very important to us and we will make every effort to see you as soon as you need us. We always have a number of 'same day sick' appointments for acute illnesses.


As your primary care provider, we manage your overall health, including acute care when you are sick, preventative care such as annual physicals and treatment for your chronic conditions. We encourage all age-appropriate screenings to be sure you are healthy and can stay that way.

Patient Centered Medical Home

Dunkirk Family Practice is proud to participate with the BCBS Patient Centered Medical Home (PCMH), a program that encourages patients to take an active role in their health. Under PCMH, your primary care provider will partner with you to assess your overall health and work with you to develop a care plan that you can both agree to and that you can manage in order to improve the state of your health.


Dunkirk Family Practice also participates with Aledade, which is an Accountable Care Organization (ACO) specifically for our Medicare patients. This program allows us to work proactively with our patients to help them get and stay healthy, not just treat them when they are sick.

Million Hearts Model

Dunkirk Family Practice is participating in Medicare's Million Hearts Cardiovascular Disease Risk Reduction Model, or the MH Model. The MH Model is a program to help people cut their risk of heart attacks and strokes.
